美国单身声明书一般由您本人来亲自起草，下面为您提供两个版本的单身声证明书（单身证明）样本，第一个版本用于非结婚目的，第二个版本用于结婚目的，您可以根据自己实际情况任选其一：“从未结婚”、“离婚”、“丧偶”。样本仅供参考，办理过程会出现各种各样的突发情况导致您被退件，我们不对退件承担责任。

美国单身证明开具好之后，还需要做领事公证认证，还需要准备以下资料：
1）公证认证申请表（需含原始签字，不接受扫描件）
2）单身声明书原件及复印件（需由您本人当着公证员签字由公证员公证）
3）申请人护照复印件及在国外居留身份证明复印件
美国单身证明领事公证认证流程：
1. 公证员公证(Notary Public)，公证员一般可以在学校、或者开户银行找到
2. 公证员所在County的县书记官（County Clerk）认证
3. 公证员所在州的州务卿认证（Secretary of State）
4. 美国国务院认证 (Department of State) 5. 中国驻美大使馆/领事馆的认证 (Chinese Embassy/Consulate)
注意：县书记官的认证（County Clerk）以及美国国务院认证这两步不是所有州都需要的。
大约15个工作日左右就可以办理完成，但是由于受疫情影响，具体办理时间不定，以使馆出件为准。
